1. Eligibility and verification
Provider accounts are available only to licensed healthcare providers and clinics operating under a prescribing physician. You represent that all credentials and practice information submitted are accurate and current, and you agree to notify us promptly of any change in licensure status.
Ordering access is granted only after verification by Elite Cell Source and may be suspended or revoked if eligibility cannot be confirmed or is no longer met.
2. Professional responsibility
All orders are patient-specific and placed under the professional judgment and authority of the prescribing provider. The provider is solely responsible for the decision to order, for the care of the patient, and for compliance with the laws and professional standards applicable to their license and state of practice.
Elite Cell Source provides an ordering and routing platform with administrative review and Medical Director authorization workflows; it does not practice medicine and does not direct patient care.
3. Ordering, review, and authorization
Submitted orders are subject to review by Elite Cell Source and, where required, authorization by the Medical Director or another approved prescriber. We may decline, hold, or return an order for more information at our discretion. An order is not accepted until it has completed review and payment.
4. Patient information
Providers agree to submit only the patient information required to process and fulfill an order, to have appropriate authority and consents for that submission, and to use the platform's patient records solely in connection with orders placed through the platform.
5. Payment
Pricing is provided within the portal to verified providers. Payment terms, timing, and accepted methods are presented at checkout or invoicing. Provider accounts may be suspended for non-payment.
6. Suspension and termination
Either party may terminate the provider relationship at any time. We may suspend or revoke access immediately where required for compliance, safety, or suspected misuse. Historical order records are retained as required by law.
