The application takes about ten minutes. You can save a draft and return to it, but having everything on hand makes it one sitting.
- Provider identity. Your full legal name — and the prescribing physician's name if you are registering for a clinic.
- NPI number. Validated for format and reviewed by our team.
- State license. License number, issuing state, and expiration date.
- Clinic information. Practice name, office address, phone, and email.
- Supporting documents. License documentation can be uploaded with your application or provided later if requested.
- Attestation. You will confirm the information is accurate and that you are authorized to place patient-specific prescription orders.
After you apply
Three things happen next.
We confirm receipt
You receive an email confirmation, and your account shows a Pending Review status.
We verify your credentials
Our team reviews your application. If anything is missing, we'll request it — your application stays open.
Access is unlocked
On approval, your dashboard opens: catalog, provider pricing, patients, and ordering.
